Create PDF Waivers and Releases from your JotForm Registrations

Running a camp or sports club can be very hectic.  You have to worry about getting people to signup online, collect payments, and make sure all the waivers and medical releases are taken care of before the first day.  JotForm can help you collect registrations and process your payments, no problem.  With WebMerge, you’ll be able to automatically generate customized waivers and releases and have them sent directly to your registrants.  How’s that for easy?

For this example, we are going to create a simple Soccer Club registration form that will collect a player’s information, parent information, and any medical conditions.  We’ve already setup the form using JotForm, here is a screenshot:

After we have the form setup, let’s go into WebMerge and setup our document. This document is going to have all of the waiver information along with some of the information that was submitted via the form.  Specifically, we’ll show the name and address of the participant.

In order to automate the process of sending data from JotForm to WebMerge, we are going to use a service called Zapier.  Zapier is a tool that allows you to connect different API’s using a simple, easy-to-use interface.  All you need to do is select JotForm as the Trigger, then choose WebMerge as the action.  Once you have authenticated both services (you can find the WebMerge API Key under Account > API Access inside WebMerge), you will want to select your Waiver document from the dropdown in Zapier.  Then the fields from your document will be loaded and all you have to do is map them to your JotForm fields.

Congrats, you’re done!  The last step will be to simply do some testing.  Go ahead and submit a registration and look for the merged PDF in your email.
